As a parent or caregiver, you need to report all of your child’s immunizations to Public Health. You can report immunizations in person at one of our locations, by mail, or online.
If you’re over 18 years old, you do not need to submit immunization information.

How do you report immunizations online?
When submitting online, make sure to upload a clear picture of your immunization record in the documents section.
Why should you report your child’s immunizations?
Health care providers do not automatically inform Public Health when they give immunizations. Reporting your child’s immunizations helps keep their records up-to-date. You may need to show an immunization record to:

Why can’t you find your immunization record online?
Your record might not be available online if:

You have not reported your immunizations to Public Health.

Your health card number has not been provided to Public Health or entered into the immunization database.
Can Public Health help with medical paperwork for school or work?
Southwestern Public Health cannot complete medical paperwork for school placements or jobs. Please book an appointment with your health care provider for this.
Can you get vaccines for school or work at Public Health?
Yes, if you know which vaccine you need, call us to discuss your options. Some vaccines are free, while others may need to be purchased through a pharmacy or your healthcare provider.
