Reporting Illness

In Ontario, child care centres are required by law under the Health Protection and Promotion Act to report certain communicable diseases to Public Health.

How to Report an Illness to Public Health

Some diseases must be reported immediately, while others may be reported the next working day. Learn more about which diseases need to be reported and how quickly by Downloading the Reportable Disease list for Ontario.

The child care centre administrator or designate should report diseases by telephone to Southwestern Public Health at 1-800-922-0096.

Be prepared to provide the following information when reporting:

  • Name of child
  • Date of birth
  • Address
  • Telephone number
  • Names of parents/guardians
  • Physician
  • Symptoms (and date
  • Name of hospital if admitted
