Reporting Illness
In Ontario, child care centres are required by law under the Health Protection and Promotion Act to report certain communicable diseases to Public Health.
How to Report an Illness to Public Health
Some diseases must be reported immediately, while others may be reported the next working day. Learn more about Diseases of Public Health Significance.
The child care centre administrator or designate should report diseases by telephone to Southwestern Public Health (SWPH) at 1-800-922-0096.
Be prepared to provide the following information when reporting:
- Name of child
- Date of birth
- Address
- Telephone number
- Names of parents/guardians
- Physician
- Symptoms (and date of onset)
- Name of hospital if admitted
